Anaesthetic drugs with a rapid onset of action and a short half-live, such as the hypnotic propofol and the analgesic opioid remifentanil, allow easy control of the anaesthetic depth and noxious stimuli associated to surgical interventions, and reveal most of the potential of target controlled infusion for general anaesthesia. Furthermore, mathematical models were incorporated in microcomputers in delivery system devices, calculating the drug concentrations in the blood and brain, according to the infusion rate. This book approaches the characteristics of propofol and remifentanil that allow the fast emergence of target controlled infusion for general anaesthesia, and its potential for closed loop control anaesthesia.

This innovative, comprehensive book covers the key elements of perioperative management of older patients. The book's chapter structure coincides with the clinical path patients tread during their treatment, from preoperative evaluation to post-hospital care. Epidemiological aspects and aging processes are illustrated, providing keys to understanding the quick expansion of geriatric surgery and defining the clinical profile of older surgical patients in a cybernetic perspective. Preoperative evaluation and preparation for surgery, including medication reconciliation and pre-habilitation, are developed in the light of supporting decision-making about surgery in an evidence-based and patient-focused way. Intra- and postoperative management are discussed, aiming to tailor anesthetic, surgical and nursing approaches to specific patients' needs, in order to prevent both general and age-related complications. This volume also addresses issues relevant to geriatric surgery, from different organizational models to clinical risk management and systems engineering applied to hospital organization.

Studies in bioequivalence are the commonly accepted method to demonstrate therapeutic equivalence between two medicinal products. Savings in time and cost are substantial when using bioequivalence as an established surrogate marker of therapeutic equivalence. For this reason the design, performance and evaluation of bioequivalence studies have received major attention from academia, the pharmaceutical industry and health authorities. Bioequivalence Studies in Drug Development focuses on the planning, conducting, analysing and reporting of bioequivalence studies, covering all aspects required by regulatory authorities. This text presents the required statistical methods, and with an outstanding practical emphasis, demonstrates their applications through numerous examples using real data from drug development. Includes all the necessary pharmacokinetic background information. Presents parametric and nonparametric statistical techniques. Describes adequate methods for power and sample size determination. Includes appropriate presentation of results from bioequivalence studies. Provides a practical overview of the design and analysis of bioequivalence studies. Presents the recent developments in methodology, including population and individual bioequivalence. Reviews the regulatory guidelines for such studies, and the existing global discrepancies. Discusses the designs and analyses of drug-drug and food-drug interaction studies. Bioequivalence Studies in Drug Development is written in an accessible style that makes it ideal for pharmaceutical scientists, clinical pharmacologists, and medical practitioners, as well as biometricians working in the pharmaceutical industry. It will also be of great value for professionals from regulatory bodies assessing bioequivalence studies.

This book is a knowledge-based text that covers the anatomy, neurophysiology, and neuropharmacology needed to carry out the necessary management and cerebrospinal protection during neuroanesthesia. Especially to make the subject of neuroanesthesia more approachable, we introduce surgical measures utilized in neurosurgery, cardiovascular surgery, neuromodulation, and related procedures. The main purpose of neuroanesthesia/neurocritical care is to deliver cerebrospinal protection in neurosurgical and cardiovascular surgeries. How should we select the best anesthetic management to prevent neurological complications for patients who, for instance, undergo different types of surgery for subarachnoid hemorrhage, stroke, head trauma, carotid endarterectomy, or cardiovascular surgery under cardiopulmonary bypass? These pathological conditions include the risk of transient cerebrospinal ischemia, and if our management is not precise, it may induce serious neurological sequelae. Although there is an urgent need to establish the treatment and elucidate the molecular mechanisms of cerebrospinal injury, many components are intertwined, making this a challenge we have not completely solved yet. This volume makes the management of neuroanesthesia more accessible not only for primary residents but also for specialists, providing a valuable resource on the current perspectives of neuroanesthesia.

The I-Gel supraglottic airway is a diposable, latex-free device for airway management during anaesthesia. The design of this device is significantly different from the previously used devices, notably laryngeal mask airways. Its bowl is made of a soft, non-inflatable thermoplastic elastomer, anatomically preformed to mirror the perilaryngeal structure. The I-Gel contains an additional epiglottis rest, which helps to prevent the epiglottis from down-folding or obstructing the laryngeal inlet. The device also includes a gastric channel, allowing drainage of air and gastric contents or passage of a gastric tube. This book provides a brief history of airway management and the classification of supraglottic airway devices; the mechanisms and forces contributing to their seal, and highlights the main differences between the devices. Also discussed is the process of I-Gel development including an initial mannequin and cadaver studies, with a description of the I-Gel's use in clinical practice, indications and description of its use in the operating room, and use of the I-Gel in different surgical subspecialities including laparoscopic procedures.
